- Starting December 27, 2021 – Regardless of vaccination status, anyone traveling to Puerto Rico by domestic flight must present a negative COVID-19 test carried out 48 hours or before arriving on the island People who do not arrive with a negative test will have 48 hours to do it in Puerto Rico or they will be fined.
- Unvaccinated people who travel to Puerto Rico on domestic flights must also quarantine for 7 days, regardless of whether they arrive with or without a negative test.

If you are heading to San Juan, Puerto Rico, you must understand the travel mandates about the COVID-19 entry requirements.
Puerto Rico’s government has a specific portal site, Travel Safe. It requires visitors to fill out the Travel Declaration Form to comply with the health department and Center for Disease Control and Prevention (CDC). This requirement applies to anyone traveling to Puerto Rico. If you do not complete the form, you will not be able to enter Puerto Rico.

Puerto Rico Travel Safe COVID Entry Requirements
All travelers entering Puerto Rico are required to provide the following:
- Complete the Travel Declaration Form
- Everyone over 2 years old must be either:
- Fully vaccinated against COVID-19
- More than two (2) weeks since you received the second dose of the Pfizer or Moderna vaccine or the single dose of the Johnson & Johnson (Janssen) vaccine.
- If you are not vaccinated against COVID-19 with the Moderna, Pfizer, or Janssen vaccines, you must provide proof of a negative molecular or antigen test performed within 72 hours before you arrive in Puerto Rico.
According to Puerto Rico’s government:
Any passenger arriving without a PCR test result for COVID-19 will be assessed a $300 fine. However, this will be waived if the traveler presents the test or evidence that a test was performed within 48 hours of arrival. This applies to all travelers over 2 years old who are not fully vaccinated against COVID-19 with the Pfizer, Moderna, or Janssen vaccines.
Furthermore, Puerto Rico’s government also issued a new executive order on November 19, 2021, to include the “booster shot” for full vaccination. If you have not seen the news, all adults 18 and older are now eligible to get another dose of the COVID-19 vaccine in the United States.
However, while at the moment the booster shot is not required to enter Puerto Rico yet, the travel declaration form is being updated to include the booster, and you may be considered “not fully vaccinated” in the near future to enter Puerto Rico if you have not received the third does of the COVID-19 vaccine. Moreover, some restaurants and lodging may not consider you fully vaccinated if you do not have the booster. So just keep that in mind as you plan your trip to Puerto Rico. Once the Travel Declaration Form is updated, I’ll update this post to include the booster information.
When to Complete Puerto Rico Travel Declaration Form
There is no set time and date that you must complete the Puerto Rico travel declaration form. You can either fill out the form online or in person at the airport upon your arrival.
I highly recommend filling out the Puerto Rico travel declaration form online a few days before your plan to arrive in Puerto Rico. After completing the form, you will be given a QR Code that you can quickly show to the airport personnel and be on your way out of the airport. On the contrary, if you are “old school” and prefer to fill the form in person, you will get in the queue that sometimes can be a long wait before you can exit the airport to enjoy your vacation.
Step-by-Step Guide to Fill Out Puerto Rico Travel Declaration Form
There are two parts when it comes to completing Puerto Rico Travel Declaration Form:
- Puerto Rico Travel Safe Registration (if it is your first time)
- Adding Puerto Rico Trip to Travel Safe
Puerto Rico Travel Safe Registration
If it is your first time using the Puerto Rico Travel Safe system, you need to register first to create your profile:
- Step 1: Go to Puerto Rico’s official Travel Safe website.
- Step 2: Scroll down until you see Complete Travel Declaration, and click on the Travel Safe button.
- Step 3: Click on the Register Here button.
- Step 4: Read, and then once you are finished, click on the Continue button.
- Step 5: Provide your personal information such as your name, date of birth, last 4 of your SSN, phone number, email address, password, etc. Then click on the Continue button.
- Step 6: On the next page, continue completing the form. You will be asked if you are vaccinated against COVID-19. You are also required to provide your home address and identity document that can be either your driver’s license or your passport.
- The system does not require you to upload your identity document. I personally chose to provide my driver’s license information.
- Step 7: Click the Continue button, and you are done registering for the Travel Safe.
- Also, don’t forget to check your email to verify your email.
Adding Puerto Rico Trip to Travel Safe
Once you successfully register for Puerto Rico’s Travel Safe, you will be able to continue to add your Puerto Rico trip to receive your QR Code.
Before you begin adding your trip to Travel Safe, make sure you have the following information handy:
- Your flight information such as airline, flight number, and arrival time.
- Your hotel/resort address, or you can google the municipality and address of your hotel or resort. If you switch resorts while in Puerto Rico, put in the first resort you will be staying at.
- You also need to provide the dates you received your eligible vaccine doses and upload your vaccination card if you are fully vaccinated.
Here are the simple steps to add your Puerto Rico trip to Travel Safe to receive your QR Code online:
- Step 1: Go to the Puerto Rico Travel Safe website and sign in if you are not already.
- Step 2: Click on Add New Trip.
- Step 3: You will be asked to provide your Trip Information such as arrival transportation, airline, flight number, arrival place, date of arrival, the purpose of your visit, where and how long are you staying, are you traveling with minors, do you have COVID-19 symptoms, etc. After you are done, simply click on the Continue button.
- Step 4: On the next page, select an option that applies to you regarding whether you are fully vaccinated or will be doing testing before traveling to Puerto Rico. You then will be prompted to supply your evidence, including uploading your vaccination record card. Click the Continue and then the Accept buttons to complete the Travel Declaration Form successfully.
- Step 5: You will be given a QR Code that you will need to show to the airport personnel before exiting the airport in Puerto Rico. There are a few ways to save your QR Code:
- You can save the QR Code in a PNG format and print it out.
- You can also check your email because the QR Code is emailed to you.
- You can also save the QR Code as a photo/screenshot on your mobile phone.
- You can also sign into the Puerto Rico Trave Safe site and click on Manage Trips, then click on your trip to get your QR Code.
- You will be able to access the “fast QR code” lane at the Puerto Rico airport when you arrive. More below.
The Process to Enter Puerto Rico With Travel Safe QR Code
Once you have received your Travel Safe QR Code, the process upon arriving and entering Puerto Rico is this:
- After exiting the baggage claim hall, you will see two checkpoints where they check your Travel Declaration information.
- Go to the QR Code lane, where your QR code and ID are checked by airport personnel. Finding the QR Code lane is a little bit confusing because it is not clearly marked, but here’s a mini direction for you.
- Based on my personal experience at the time of this writing. Within the baggage hall, you walk all the way to the end of the hall, where you see Exit & Rideshare.
- The line there (should be on your right) is what you want to have your QR Code and your ID checked. The other line on your left-hand side can be a lot longer for those who do not have the QR Code.
- Your QR code will be scanned. Your name and the picture of your vaccination record card come up on their mobile phones. Then they also need to scan your ID, so make sure to have that ready.
- Once they are verified, you are good to go!
